This form is required when a customer is requesting transaction history on transactions sent or received. NEW ID Card and Proof of Address are required to be submitted with this form.

Customer reinstatement form

This form is required when a customer is advised that he /she is no longer allowed to use the service.

Declaration of source of funds form

This form is required for individual or cumulative transactions of $7,500 BBD or more. NEW ID Card is required to be submitted with this form.

Registration for direct to bank form

This form is required for customers who want to have received transactions deposited to their Bank/ Credit Union account. NEW ID Card and front page of Bank/Credit Union Statement are required to be submitted with this form.

non payment claim form

This form is required when a transaction sent have been reported as "not received" but recorded in the Western Union system as "paid". NEW ID Card is required to be submitted with this form.
